Build a custom survey template at Surveys → View Templates → + Create Template. Give the template a name, type, and tags, then add questions from the question library.
Open the builder via Surveys → View Templates → + Create Template
Set a Template Name, optional Short Name, Survey Type, and Tags
Add a Description so other admins know when to use this template
Build the question set using + Add Question to pick from the question library
Save as Draft while building; publish when ready to use
Opening the builder
From the admin sidebar, click Surveys (under Detection).
Click View Templates in the top right of the Surveys page.
Click + Create Template in the top right of the Survey Templates page.
The Create Custom Template page opens with two main sections: Survey Details (template metadata) and Questions.
Survey Details
Field | What to enter |
Template Name (required) | The full name of the template, for example "Employee Wellness Survey" |
Short Name | A short display label (max 100 characters; auto-generated if left blank) |
Survey Type (required) | Pick from the dropdown; the type filters which questions are available to add |
Tags | Free-text tags for searching and filtering. Press comma or Enter to add a tag |
Description | A short explanation of when this template should be used |
Adding questions
Click + Add Question in the Questions section. The library is filtered by your selected Survey Type.
Each question maps to one or more weighted tags that link responses to dimensions, hazard categories, and protective factors. Scoring rolls up automatically as responses are submitted.
You can:
Reorder questions by dragging
Edit or remove questions individually
Add as many questions as you need; the builder shows the running count
Saving and publishing
While building, click Save Draft at any point. Cancel exits without saving.
Once the template is complete, publish it from the template detail page. Published custom templates appear under the Custom tab on the Survey Templates page and can be selected when creating a new survey.
