Opening the builder
From the admin sidebar, click Surveys.
Click View Templates in the top right of the Surveys page.
Click Create Template in the top right of the Survey Templates page.
The Create Custom Template page opens with two sections: Survey Details (the template's metadata) and Questions.
Survey Details

Type shapes the questions: Your chosen Survey Type filters the question library. Today the only type is Psychosocial, so the library surfaces psychosocial questions. Set the type before you start adding questions.
Adding questions
Click Add Question in the Questions section. This opens the Question Library ("Select questions to add to your survey"), a pre-built, pre-tagged set of validated questions. You choose from the library rather than writing questions yourself.
Filter the library by:
Survey type (Psychosocial)
Subcategory: the specific work factor a question measures (for example Fatigue, Job demands, Low job control, Harassment)
Type: the question format
Each question is mapped to one or more weighted tags that connect a response to the psychosocial work factors and protective factors it measures. Scoring rolls up automatically as responses come in; you don't score anything by hand.
Once questions are added you can:
Reorder them by dragging
Edit or remove them individually
Add as many as you need; the builder shows a running count
All library questions today use a single format: a Likert scale (a graded agreement or frequency scale). There are no yes/no, multiple-choice, or free-text questions.
Saving and publishing
While you build, click Save Draft at any point (Cancel exits without saving). When the template is complete, publish it from the template detail page. Published custom templates appear under the Custom tab on the Survey Templates page and can be selected whenever you create a new survey.
