Choosing a survey template

ReFresh ships with platform templates aligned to the 17 psychosocial hazard categories. Pick a platform template if you want a regulator-aligned survey out of the box, or build your own custom template (see article 3.3).

  • Open the template picker via Surveys → View Templates

  • Templates are organised under three tabs: All Templates, Platform, Custom

  • Platform templates are published by ReFresh and aligned to regulatory frameworks

  • Custom templates are templates your organisation builds and reuses

  • Pick a template when creating a new survey via Surveys → + Create Survey


Where templates live

From the Surveys page (Admin sidebar → Detection → Surveys), click View Templates in the top right. The Survey Templates page shows three tabs:

  • All Templates: every template available to your organisation

  • Platform: published templates ReFresh provides

  • Custom: templates your organisation has built

Each template shows its name, category, status (published or draft), source (Platform or Custom), and last updated date. Use the search bar or category dropdown to filter.


Platform templates

Platform templates are aligned to the 17 psychosocial hazard categories and 11 protective factors. They are published, validated, and ready to send. The default platform template is Psychosocial Risk Survey - Condensed, designed for quick high-signal reads.

Use a platform template when:

  • You want a regulator-aligned starting point

  • You do not need to customise the question set

  • You want results that compare cleanly against other ReFresh tenants and against framework expectations


Custom templates

Build a custom template when you need a survey that reflects your organisation's specific context, language, or priorities. See "Building a custom survey" (3.3).

Once published, custom templates appear in the same picker as platform templates.


Picking a template when creating a survey

When you click + Create Survey on the Surveys page, the Create New Survey dialog asks you to:

  1. Pick a Survey Template from the dropdown

  2. Give the survey a name (for example, "Q3 2026 Psychosocial Health Assessment")

  3. Optionally add a description

Click Create Survey to create the survey in draft state, then add recipients and send.
