Reporting an incident (admin view)

Incidents capture events that have happened, including worker reports submitted through My ReFresh and admin-logged incidents.

Incidents capture events that have happened, including worker reports submitted through My ReFresh and admin-logged incidents. Open Incidents at admin sidebar → Detection → Incidents, with sub-pages for Triage Queue, All Incidents, and Investigations.

  • Incidents live at sidebar → Detection → Incidents

  • Three sub-pages: Triage Queue, All Incidents, Investigations

  • Workers can submit incidents through My ReFresh; admins can log incidents directly

  • A shareable reporting link can be generated for anonymous and identified worker reporting

  • Each incident has a code (for example, INC-2026-0004), category, type, severity, and lifecycle status

Triage Queue

Incidents waiting to be classified and routed to the right team. Admins reassign each incident to a Compliance Manager or other admin

All Incidents

Every incident in your organisation. Status bar at the top shows Reported / Investigating / Resolved / Closed counts

Investigations

Active and completed investigations linked to incidents (see "Investigating an incident", 4.7)


Logging an incident as an admin

From All Incidents, click + Report New Incident. The incident form captures:

  • A short summary

  • Category (Psychosocial, Physical, Safety, Conduct, Other)

  • Type (Near Miss, Complaint, Conduct, Something Else)

  • Severity (Catastrophic, Major, Moderate, Minor)

  • Date, time, and location

  • The people involved

  • A description of what happened

  • Whether a regulatory notification is required

Save the incident; it appears on All Incidents in Reported status.


Sharing the reporting link

From the All Incidents page, click Share Reporting Link. This generates a link your workers can use to submit incidents. The link supports both identified and anonymous reporting.

Share the link in:

  • Onboarding materials

  • Posters in shared spaces

  • Internal communications

  • Email signatures for managers

Triaging incidents

Open the Triage Queue to see incidents that have not yet been classified. For each incident:

Anonymous reporting flag if applicable

Click Reassign on any incident to assign it to a Compliance Manager or admin. Once classified, the incident moves out of the queue and into All Incidents under its assigned investigator.


Incident lifecycle

Reported

Incident logged but not yet under active review

Investigating

Investigation has started

Resolved

Corrective actions complete; incident ready to close

Closed

Incident formally closed; record retained for audit trail

The status bar at the top of All Incidents shows the count in each state.
