Reporting an incident (worker view)

Workers report incidents via Incidents → **Report Incident** in My ReFresh. The wizard has four steps: What Happened, Details, When & Where, Review.

Workers report incidents via Incidents → Report Incident in My ReFresh. The wizard has four steps: What Happened, Details, When & Where, Review.

  • Open at sidebar → Incidents → Report Incident

  • Four-step wizard with Anonymous toggle at the top

  • Step 1 captures a title and description

  • Step 2 captures incident type, severity, and history

  • Step 3 captures when and where

  • Step 4 reviews everything and asks you to acknowledge a privacy statement before submitting


Step 1: What Happened

Two fields:

  • Incident Title (required): a short summary of what happened

  • Description (required): the full account in your own words

Be as specific as you can: include names of people involved, witnesses, and any immediate actions taken. The description supports paragraph-length text.

Click Continue to move to Step 2.


Step 2: Details

The wizard analyses your description and pre-fills suggestions where it can. You can override anything it suggests.

Incident type (required, pick one):

  • Complaint: I want to raise a concern about something at work

  • Injury or Illness: someone was hurt or became unwell

  • Near Miss: something almost caused harm but did not

  • Something Else: it does not fit the above

Workplace hazard type (optional): pick a category if you know which type of workplace hazard the incident relates to.

Severity (required, pick one):

  • Insignificant: negligible, no injury or impact

  • Minor: minor, no one was hurt

  • Moderate: some impact, discomfort or disruption

  • Major: serious, someone was hurt or at risk

  • Catastrophic: emergency, life-threatening or severe

Has this type of incident happened before?: No / Yes / Not Sure.

There is immediate risk to myself or others (checkbox): tick this to flag the report for immediate attention.

Click Continue to move to Step 3.


Step 3: When & Where

Field

What to enter

This incident is ongoing / active (toggle)

Tick if the situation is still happening or unresolved. The wizard may suggest this based on your description

When did this occur? (required)

Date and time

End Date (optional)

If the incident spanned a period, when did it end?

The time is approximate (checkbox)

Tick if you do not remember the exact time

Where did this occur? (required)

Location, for example "Main Office, Level 3, Meeting Room B"

Additional Location Details (optional)

Any extra detail that helps locate where the incident happened

Click Continue to move to the Review step.


Step 4: Review

The Review step shows everything you have entered, grouped under What Happened, Classification, and When & Where. Use Edit on any section to go back and change something.

Below the review, the privacy collection statement explains how your information is handled. Two acknowledgements are required:

  1. "I have read and understood how my report and personal information will be handled."

  2. "I confirm that the information provided is accurate to the best of my knowledge. I understand that making false or malicious reports may result in disciplinary action."

Tick both, then click Submit Report.


After you submit

Once submitted, the incident appears on your My Incidents page with status Reported.

For identified reports, the detail page shows an Identity Shared notice: "Your name and account are visible to the team reviewing this report."

The "What happens next?" section explains: your report has been received and is awaiting review by your HR or WHS team. You will see status updates here as the review progresses.
