The Quarterly Psychosocial Risk and Culture Report

The Quarterly Psychosocial Risk and Culture Report is the primary board-ready document ReFresh ships, pulling compliance posture, framework progress, control effectiveness, records status, incidents, and risk trends into one report.

Written by Taylor Laing
  • Generated from the Quarterly Psychosocial Risk and Culture Report template on the Reports page

  • Default reporting period is a calendar quarter

  • A ~7-page document (editable): executive summary, metrics, trends, and supporting detail

  • Fully editable: customise sections and add your own commentary

  • Exportable as PDF or PowerPoint (PPTX)

Creating the report

This report is a template on the Reports page. Create one exactly like any other on-demand report:

  1. Go to the admin sidebar → Governance → Reports.

  2. Click New Report and choose the Quarterly Psychosocial Risk and Culture Report template.

  3. Name it and click Create to open the draft in the editor.

The generated report shows Type = Custom (a freely editable instance of the template). For the full editor walkthrough, see Generating an on-demand report.

What the report covers

This report is built to answer, in one document, "how are we managing psychosocial risk?" It draws on your compliance posture, framework progress, control effectiveness, records status, incidents, and risk-register trends. The cover carries the report title, the Reporting Period (for example "Q4 2025"), and Prepared by (your organisation).

The report ships with a default seven-page structure, organised around the Detect, Controls, and Manage pillars and framed by a cover and executive summary:

  1. Cover: the report title, Reporting Period, and Prepared by.

  2. Executive Summary: Report Purpose, Key Board-Level Observations For This Period, and Key Psychosocial Risk Indicators.

  3. Detect: High Risk Detection Analysis, Medium Risk Detection Analysis, and Worker Consultation Findings.

  4. Controls: The Psychosocial Hierarchy of Controls Framework, and Policy Updates.

  5. Manage: Incident Reporting Detailed Analysis, Incident Breakdown and Root Cause, and Ongoing Management and Future Focus.

Because the report opens in the drag-and-drop builder, this structure is a starting point rather than a locked one: you keep, edit, remove, or reorder sections to suit the audience.

When to use it

Reach for this report for:

  • Quarterly board meetings

  • Executive committee reviews

  • Internal compliance reviews

  • The cover document in a regulator evidence pack (see Compiling an evidence pack for a regulator)

For ad-hoc reporting on a single module, use a custom on-demand report instead (see Generating an on-demand report).

Customising the report

Open the report in the editor to tailor it:

  • Adjust the reporting period

  • Add or remove sections

  • Add your own commentary with the Text panel

  • Swap charts for versions tailored to your audience

  • Add an executive note or summary page

Keep it in Draft while you iterate, then Mark as Ready when it's final.

Exporting

From the editor or the Reports list, export the finished report as:

  • PDF: the standard board-distribution format

  • PowerPoint (PPTX): when you're presenting it in the meeting
