Records and document upload issues

If a Record fails to upload (a document, policy, or test evidence file) the cause is usually file type, file size, or something browser-related.

Quick checks:

  • Upload common document formats (for example PDF or Word); if a file is rejected, convert it to a widely supported format.

  • Very large files can fail or time out; split them into multiple Records where possible.

  • Try a different browser if an upload stalls.

  • Check the file isn't corrupted; open it locally first.

Supported file types and size limits

Each file can be up to around 10 MB. Accepted types include .pdf, .doc, .docx, .xls, .xlsx, .png, .jpg, .jpeg, .gif, .eml, and .msg. If a file is rejected as an unsupported type, convert it to one of these formats, or reference it by a link instead of uploading it. If a file is over the size limit, compress it or split it across multiple Records.

Documents showing as expired

If a Record shows as expired:

  • Its expiration date has passed (see Policy version control and renewal tracking).

  • Upload a new version with a fresh effective and expiration date.

  • The previous version is retained in version history; the new one becomes the current evidence.

Submitting a Record for review

After uploading a Record, submit it for review to send it to the relevant reviewer. Until it's submitted and approved, it doesn't count towards the control it supports.
