Evidence and document upload issues

Troubleshoot problems uploading evidence and documents: file size limits, unsupported formats, upload failures, and storage quota issues.

If an evidence file or document fails to upload, the cause is usually file type, file size, or browser-related.

  • Supported file types: PDF, DOCX, TXT, MD

  • Maximum file size: 500 MB per file

  • Try a different browser if upload stalls

  • Check that your file is not corrupted (open it locally first)

  • For very large files, split into multiple evidence items if possible


Supported file types and size limits

ReFresh accepts:

  • PDF (.pdf)

  • Word documents (.docx)

  • Plain text (.txt)

  • Markdown (.md)

Unsupported types (.xlsx, .pptx, .jpg, .png, .zip) need to be either converted to one of the supported types or referenced via a link instead of uploaded.

The maximum file size is 500 MB per file. Larger files need to be split.


Common upload issues

Symptom

"Unsupported file type" error

File extension not supported

Convert to PDF or DOCX

Upload stalls or times out

File too large or slow connection

Split the file or try on a stronger connection

Upload completes but file shows as corrupt

File was corrupt before upload

Re-export the file from its source

Upload completes but evidence shows as Draft

Normal: evidence is saved as Draft until submitted for review

Click Submit for Review when ready


"Documents showing expired"

If a document or evidence item shows status Expired:

  • The document's expiration date has passed (see "Policy version control and renewal tracking", 4.11)

  • Upload a new version with a fresh effective and expiration date

  • The old version is retained in Version History; the new one becomes the current evidence


Submitting evidence for review

After uploading evidence as Draft, click Submit for Review to send it to the relevant reviewer. Until submitted and approved, the evidence does not count as Complete on the control.
